Centralizing Disability-related and Medical Documents
Any documents regarding a University employee's disability, injury, or prolonged illness must be centralized in Disability Services rather than in the Office of Human Resources or any other location on campus. Departments or individuals should not keep any copies of such documentation within departments or offices.
- Any existing information related to an employee's disability, including medical reports, should be forwarded to Disability Services, University of Minnesota, 230 McNamara Alumni Center, 200 Oak Street S.E., Minneapolis, MN 55455.
- Any new information acquired about a disability, injury, or prolonged illness must be requested by Disability Services. If new information is inadvertently received by a department, it should be forward to Disability Services.
An office specialist in a department was diagnosed with severe chronic depression, and although treatment had been initiated, she continued to experience bouts of crying during times of stress. Solution: After discussion with her supervisor, she was provided with the use of a small room, where she could use the private area to compose herself.
